Our office hours for scheduling appointments, prescription refills and general questions are 9am-5pm Mon-Friday, except for major holidays.

Insurance Information

New patients should arrive at least 30 min prior to their scheduled appointment to complete paperwork, please bring a photo ID, insurance card and all medications. All major insurances are accepted, if a referral from your family doctor is required by your insurance provider, please confirm that we have that prior to your arrival.

WHAT TO BRING

To streamline your visit and minimize waiting time we ask patients to bring these items to your appointment:

1. Picture ID and insurance cards

2. Completed paperwork if obtained from website

3. Any medical records

4. All medication bottles(with intact labels) and Medication list with allergies

5. Any Xrays and MRIs including reports if available

6. Pharmacy information

7. Valid referral if needed.
